Tapestry 360 Health is a nonprofit, federally qualified health center (FQHC) with 15 locations serving Chicago’s North and Northwest Sides and nearby suburbs. We provide more than 28,000 patients each year with a trusted medical home, offering affordable, comprehensive primary and preventative care, behavioral health services, and other integrated services designated to support lifelong wellness.
As a patient-centered medical home, Tapestry 360 Health delivers coordinated, high-quality, culturally responsive care that addresses each person's unique health needs.
